The NREMT and many state EMS boards require a disclosure of criminal and/or a background check be performed in order to become a licensed/certified EMT. I understand that as part of the course of instruction, I may be placed in close contact with other students, instructors and actual patients. Such contact may result in my contact with diseases and human blood or bodily fluids, and/or items contaminated with blood or bodily fluids. I understand that Allied Medical Training strongly encourages receiving a full dose of the COVID-19 vaccine for my own health and safety during in-class training. I agree to practice standard precautions to protect myself from contact with potentially contaminated items. By my signature on this application, I hereby release and agree to hold harmless Allied Medical Training, LLC, as well as any hospital or healthcare agency where clinical training is completed and their respective employees and agents from any and all damages that might result from such contact. I further understand, that while it is not a requirement for admission to or participation in this course, all students are urged to seek out and obtain vaccinations against Hepatitis B prior to attending the course. Further, I understand that as part of the course I may be performing physical activity and that the course may also require the use of medical equipment. By my signature below, I hereby certify that my physical condition will allow me to participate without risk to myself or others and I release and agree to hold harmless Allied Medical Training, LLC as well as other clinical training sites and their respective employees and agents from any and all damages which might result from such activity or use.
I understand that if I only sign up for the online component of the course, I have 6 months of access. When the 6 month deadline is reached and the course is incomplete and no registration for skill training (Skill Week) has been made, or the course completion requirements were not completed by the deadline for their chosen skill training (Skill Week), access to the course may be terminated without notice.
I understand that in order to successfully complete this part of the EMT course I must, unless otherwise stated in the course syllabus, attend all required online sessions and obtain a passing score on each quiz, exam, and the final exam. I understand that not achieving a passing score on an assessment will prevent me from moving forward in the course and result in my course withdrawal.
In addition, to successfully complete the entire Self-Paced EMT course I must pass the online part of the course, skill training (Skill Week), and the overall course practical skills exam conducted by Allied Medical Training, LLC and a standardized patient clinical rotation performed through Allied Medical Training, LLC. Successful completion of this course will make me eligible to become certified as a National Registry Emergency Medical Technician (upon successful completion of NREMT written exam). I understand that the NREMT written exam is a standardized external examination and that Allied Medical Training, LLC cannot guarantee a passing score on this exam.
I understand that if I do not pass any component of the course at any point, I cannot re-enroll in this course. I understand I will have to complete a full EMT initial course with a different program if I still want to pursue EMT training.
I understand that I am responsible for finding out my own state’s requirements for becoming a certified EMT to verify that they will accept NREMT certification.
The following statement applies if your course fee was sponsored by your employer or any other organization: I consent to have my education records, including but not limited to my grades and attendance reports, provided to my immediate supervisor and upper level management of my employer or my caseworker and the organization that helped me enroll in the course. I understand that payment must be received by one month after my enrollment begins or by one month before my in-class (Part 2) training – whichever is sooner. I understand my enrollment in the course will be cancelled if payment isn’t received by the deadline.
