By submitting an application, the applicant understands and agrees to all of the following terms and conditions without exception:
This is a non-credit class. No university academic credit will be issued.
Allied Medical Training is not an eligible educational institution for the purposes of reporting tuition on income tax to receive tax credit.
In the rare event of course is cancelled, you will receive a full refund.
If you withdraw or transfer prior to 5 business days before first class: $100 fee
If you withdraw or transfer within 5 business prior to first class: NO refund
After class has begun: NO refund
Failure or withdrawal from the class: NO refund
Course size will be determined prior to the start of the class. If insufficient completed applications are received by the close of business on or prior to the deadline date, the course may be cancelled and complete reimbursements will be distributed to all applicants. Students will be accepted on a first come, first served basis.
Applications will be accepted until the course is full.
CPR certification at the Healthcare Provider level is required by Minnesota law to become a certified EMT. If you don’t have this certification, Allied Medical Training provides training sessions at additional cost during the second week of EMT class. Alternatively, you may obtain this certification through any AHA-certified instructor.
Tuition and Fees:
TOTAL TUITION: $1,750
Tuition is due in these installments:
– Due Upon Registering: $250
– Due on First Day of Class: $1,500
Textbook: You must obtain a print copy or digital access to the textbook by the first day of class. Information on where to find this textbook for sale will be sent to you in the confirmation email. Prices typically range from $50 – $110 depending on your choice of new or used print, or digital.
CPR Certification (if needed) fee: $80 due on first day of class.
The $250 deposit to reserve your place in the class will be deducted from the total tuition. The remaining tuition plus applicable textbook fee is due on or before the first day of class (or no textbook fee to us, if you choose to purchase the eBook). Payment is accepted via cash, check, credit/debit card, or Paypal. Instructions to submit your final class payment will be sent to you before the class begins (you may bring final payment to your first day of class).

Tuition is NOT refundable after the start of the class for any reason including withdrawal or the unlikely event of failure of the course.
A returned check fee of $20 will be assessed on all returned checks.
I understand that as part of the course of instruction, I may be placed in close contact with other students and actual patients. Such contact may result in my contact with diseases and human blood or bodily fluids, and/or items contaminated with blood or bodily fluids. I agree to practice standard precautions to protect myself from contact with potentially contaminated items. By my signature on this application, I hereby release and agree to hold harmless Allied Medical Training, LLC, as well as any hospital or healthcare agency where clinical training is completed and their respective employees and agents from any and all damages that might result from such contact. I further understand, that while it is not a requirement for admission to or participation in this course, all students are urged to seek out and obtain vaccinations against Hepatitis B prior to attending the course. If required by clinical rotation site, I agree to provide proof of current negative tuberculosis status and proof of current vaccination against Measles, Mumps and Rubella. Further, I understand that as part of the course I may be performing physical activity and that the course may also require the use of medical equipment. By my signature below, I hereby certify that my physical condition will allow me to participate without risk to myself or others and I release and agree to hold harmless Allied Medical Training, LLC as well as other clinical training sites and their respective employees and agents from any and all damages which might result from such activity or use.
Background Check Disclosure:
The NREMT and many state EMS boards require a disclosure of criminal and/or a background check be performed in order to become a licensed/certified EMT. If you are concerned you may not qualify for a license based on a previous conviction or arrest, please contact the NREMT and your state EMS office to verify your eligibility.
I understand that in order to successfully complete the course I must attend all required online and in-class sessions and obtain a total score of 75% or above on all assessments and 71% on the final comprehensive exam.
In addition, to successfully complete the course I must pass both the overall course practical skills exam conducted by Allied Medical Training, LLC and a clinical rotation taken through an arranged clinical site or standardized patient clinical rotation performed through Allied Medical Training, LLC. Successful completion of this course will make me eligible to become certified as a National Registry Emergency Medical Technician (upon successful completion of NREMT written exam). I understand that the NREMT written exam is a standardized external examination and that Allied Medical Training, LLC cannot guarantee a passing score on this exam.
